IMPORTANT – Once we book your appointment you would then receive an email notification. Within that email, you will find links to two online forms and instructions on how to pay for your appointment. To confirm the booking all new patients now require to make a payment and submit the forms within 24 hours of receiving the appointment notification email. New patients are required to complete both New Patient and COVID-19 Triage forms. Existing patients need to complete just the COVID-19 Triage form. Within the email, you will also find information on how to find us, where to park, our current procedures. If you don’t receive this notification email, please check inside your email’s spam/junk folder, if you haven’t located the email, please get in touch with us.
Failure to submit the forms will result in the cancellation of your appointment.
